Incident Reporting

If an incident occurs during a hike, trail clearing or at the clubhouse, please fill out an incident report:

Safety Matters.  HTMC is committed to the safety of both our participants and volunteers. The Incident Report Form is one risk management tool that helps minimize risks. Reviewing incident reports helps us prevent future incidents and help us learn and improve our safety practices. 

In addition to having Hike Coordinators and Trail Clearers report any safety incidents, we are also encouraging our HTMC members to help us accurately report any incidents.  As a witness to an incident, your observations and concerns are encouraged.

On a Hike 

Since a Hike Coordinator might not be immediately at a scene of an incident, as a member of HTMC you can help us by collecting information and then submitting this info to the Hike Coordinator. We recommend that you bring a paper version of the form so that you can collect the needed information. As a witness, you can also submit your own Online Incident Report Form. 

 
When should an Incident Report Form be filled out and submitted to the HTMC Coordinator.

The simple answer: for any incident that requires more than a bandaid. 

Was any first aid care needed? 
Did the incident hinder participation in the activity?
Was there a need for follow-up care after the activity? 
Was there an evacuation?
Was there a crime? (vehicle break ins)

Filling out the Incident Report Form. 

If an incident happens, once things are stable, start filling out the Incident Report Form.  

Get contact info for those involved in the incident, including witnesses. 

Get information about what happened/caused the incident. Use the involved person(s) own words. If the Hike Coordinator was not present, get witness statements. These statements should be in the witnesses’ own words.

Also record what steps were taken right after the incident. Who did what. And please share any thoughts from those involved, witnesses, and yourself as to how to prevent similar incidents or how to be better prepared. Any lessons learned? 

As soon as possible, submit the information to the Hike Coordinator.  

Paper and Online Incident Report Form

There is both a paper and online version of the Incident Report Form. The Online Form is the final form that needs to be submitted for all reportable incidents. As a HTMC member we recommend that you submit your info to the Hike Coordinator, who will then submit the online form. And you can also submit your own Online Incident report form.

You can decide to collect the information and directly input into the online form or you can collect the information on the paper form and then transfer the data to the online form. You can bookmark the link for the online form on your phone.
